Rev.
Jerry Wiseman agrees to provide Officiator service on _______________________,
20______, with the wedding ceremony starting at _____________________ (time)
for the below named Bride and Groom.
Rev. Wiseman will arrive at the below ceremony location 30 minutes prior
to the ceremony time and will stay up to 30 minutes following the ceremony time
(maximum of 2 hours). Additional time
exceeding 2 hours will be charged at $25.00 per 15 minutes. The Bride and Groom recognize that Rev.
Wiseman may schedule more than one wedding ceremony per day and may not be able
to accommodate long delays due to another commitment.

In
the event that the full amount is not paid by ___________________, the ceremony
date and time will be automatically cancelled and the $50 deposit
forfeited.

will provide Rev. Wiseman with the marriage certificate at the time of his
arrival to the ceremony to allow time to fill out necessary paperwork. The Bride/Groom agrees to ensure that the
marriage license will be available prior to the ceremony and understand Rev.
Wiseman will not perform the ceremony without the marriage license.
Once signed
below, no changes can be made in this agreement and in the event of a
cancellation the $50 deposit will not be refunded. If the Officiant is
unable to perform the ceremony due to unforeseen circumstances such as
death, sickness or accident beyond the Officiant’s control, all monies received
will be returned to Bride/Groom. This contract is therefore considered by
both said parities to be legal and binding in accordance to the conditions set
forth herein:
